ADDITIONAL REQUIREMENTS FOR ON-HIGHWAY EQUIPMENT

• Full Automobile liability insurance with combined single limits of not less than One Million Dollars ($1,000,000.00).
• Kelly Tractor Co. must be listed as an additional insured.
• Policy exclusions, for any reason, must be fully identified and appear on the insurance binder, and/or certificate. Some exclusions may not be acceptable.
• Certificates of insurance must be provided prior to delivery of the equipment. NO EXCEPTIONS.
• Please be aware of commercial driver’s license (CDL) requirements for water trucks.
• NO FTV (Fire Theft Vandalism) is available. Customer to bear all risk of loss. Make sure to cross this out on Rental Agreement.
